Original passport. Original passport must be valid for six months beyond exit date and have two blank facing visa pages.

Passport-type photograph. One passport-type photograph.

Proof of status. Original evidence of your legal status in Australia (Original work, student, or resident type visa). Electronic visa holders should submit their Australian entry record or a residency confirmation issued by DIAC.

Itinerary. A copy of the flight confirmation showing onward/return travel including applicant’s name. If more than one entry and exit is required then the confirmation must reflect the number of entries and exits required.
